PAR-Q & Medical Clearance & Health History:

A medical clearance form is required of all participants who answer “yes” to any of the seven PAR-Q questions. Note: Personal training staff reserve the right to require medical clearance from any client they feel may be at risk or a liability. A liability waiver form for exercise testing as well as physical fitness program are required and signed by both trainer and participant before starting any personal training program at PI4L Fitness & Nutrition, LLC.

Session Duration:

Personal training sessions are (60) or (90) minutes in duration. Training may also be (30) minutes in length and will count for half of a session or an add-on (not applicable on the 12 weeks to a new you program,or Partner package purchases).

Training Attire:

Please come prepared to each training session in clean and proper workout attire and footwear (shorts, gym pants, T-shirt, sweatshirt, supportive sneakers).  Participants arriving unprepared for their training session will  lose the training session. A credit may be provided at the discretion of your personal trainer.

Returning Clients:

At PI4L Fitness & Nutrition LLC we live by the motto, once our private client, always our private client. When you return to training within a (3) year period your original PARQ, Health History Forms, Liability Release, etc. will be used as long as your personal information is still accurate from the last time that you worked with your personal trainer at PI4L Fitness & Nutrition, LLC. A new contractual agreement for personal training services will be created to reflect new sessions duration and services rendered.

Late Policy:

Training sessions are based on a (3)day a week schedule and failure to make a session for whatever reason is still subject to a charged session. Clients are responsible for arriving on-time to their training sessions. Trainers are obligated to wait only 15 minutes (10 minutes for 30-minute sessions). After 15 minutes (10 minutes for 30-minute sessions), the trainer is not required to lead the remaining time of the session and the session may be lost or rescheduled at the trainers discretion.

Gift Accounts:

All accounts purchased by someone other than the training participant as a gift or otherwise are NON-REFUNDABLE and NON-TRANSFERABLE. All un-used training sessions in a gift account are forfeited if participant does not or cannot fulfill said training obligation and agreement.

Session Cancellation Policy:

Clients are asked to call (24) hours in advance of the scheduled training session. You will be charged for appointments cancelled with fewer than (24) hours notice. Failure to contact your trainer through text or call 702-341-6052 (M-F) will result in a session loss.

Program Cancellation/interruption Policy:

Temporary Interruption of service requires a written request to PI4L Fitness & Nutrition, LLC stating reason for interruption and anticipated continuation. Cancellation of program and/or services requires a (30) day written notice to avoid charges for sessions already scheduled. Notice of cancellation will only be valid when hand delivered or sent by certified mail at the address for notices as defined below. In the event the client terminates said agreement, PI4L Fitness & Nutrition, LLC shall retain all payments made for all unused training sessions, (PIF)Paid in full contracts and/or any unused portion of a one-to-one personal training or meal planning program.

Package Expiration/Refund Policy:

Individuals registering for personal training must complete all personal training sessions by the end of the client’s eligibility to participate in the program (current or by the expiration date of the training package, whichever comes first. All personal training packages expire one (1) month or (30) days after end date on the client agreement form from the orignal date of purchase. All Private Personal Training Packages and Meal Planning Services are NON-REFUNDABLE/non-transferable regardless of the situation. This includes all moneys paid upfront for personal training and meal planning services and all (PIF) paid in full contracts. Please refer to the Service Agreement, Refund, and Credit Policy for current policy information posted on the wall in our private personal training studio or in your client folder for your convenience.

Credits for personal training may be granted for future training sessions.  Absolutely NO REFUNDS on personal training, meal planning packages, (PIF) Paid In Full contracts are given. There will be absolutely no exceptions.
